Insurance:

Services may be covered in full or partially by your health insurance or employee benefit plan.
Please check your coverage carefully by asking the following questions:

Do I have mental health insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?

Payment Policy:

A full payment- or any copay- is required at the time of service. Credit cards are accepted. Please remember that you are responsible (and not your company, attorney, or adjunct service provider) for paying the fees agreed upon.

Cancellation Policy:

My practice maintains a waiting list for those who desire an appointment but cannot get in due to the full schedule.  Consequently, if you break your appointment, you have chosen to give up your reserved hour.  Therefore, if you do not attend your scheduled appointment, and you have not notified me 24 hours prior to your appointment (or, by noon on Friday for Monday appointments), you will be required to pay the full cost of the session or evaluation time as you have reserved it. 
Please note, you will never be charged for breaking an appointment in the case of a true emergency beyond your control.
